The cause was a staff shortage due to extended leave without timely replacement. The holder accepted the recovery offer and submitted an action plan with structural roster management, early deployment of the flex pool, potential group closures, and active recruitment. Quality managers revised the assessment framework to stop 'shifting' between locations. Following these measures, the GGD considers the violation to have ended and advises against an administrative sanction.
